Below you will find the 2011-2012 student athlete forms.
The medical release form, emegency card, player CIF code of ethics and WCHS parent code of ethics MUST be completed by the player, parents AND/OR the doctor , in order for you to participate in any athletic program during the 2011-2012 school year. YOU WILL NOT BE ALLOWED ON THE FIELD OR COURT WITHOUT THESE FORMS, SO YOU WILL WANT TO GET ON THIS RIGHT AWAY AND TURN THEM IN!
